Patient Forms

If you are new to our practice, we require a completed Health History and Acknowledgement of Privacy Practices prior to your first dental visit.  For your convenience, you may print, review, and complete these forms at home before your appointment, and bring the completed documents with you on the day of your visit.  If you prefer, however, you are welcome to review this information and complete the forms in our office immediately prior to your appointment.
